POSITION SUMMARY:

The consultant pharmacist interacts with professional staff, technical staff, lay staff and clients in an effort to promote and maintain superior standards of quality that is professionally acceptable to Skilled Nursing Pharmacy and the consumers that are served by Skilled Nursing Pharmacy. In addition, the consultant pharmacist will assist the consultant pharmacist manager and management in implementing regional objectives as determined and directed by the governing body.

The consultant pharmacist will be responsible for performing drug regimen reviews and nursing unit inspection. He/she will participate at the long-term centers Quality Assurance Committee, infection control committee, and other committees requiring a pharmacists expertise. The consultant will provide pharmacist-to-nursing in-services. The consultant pharmacist will review with the Director of Nursing and the Administrator all relevant changes in state and federal pharmacy regulations and changes in Skilled Nursing Pharmacy policy and procedures. The consultant pharmacist will serve on the implementation team of their assigned center. The consultant pharmacist must attend consultant pharmacist meetings and work the schedules proposed by the consultant pharmacist manager.

WORK AND DRIVING REGION: Merced, Modesto and Stockton Area

ESSENTIAL DUTIES:

  • Provide regular and timely consultant services to client facilities and make recommendations as appropriate.
  • Review patient drug regimen in assigned facilities.
  • Report any drug regimen irregularities to the Attending Physician and Director of Nursing.
  • Provide physicians, nurses, and patients with therapeutic recommendations and/or medication information.
  • Establish a system of records of receipt and disposition of all controlled drugs in sufficient detail to enable an accurate reconciliation.
  • Assist client facilities in developing and implementing policies and procedures
  • Actively participate in client facilitys quality improvement process and attend facilitys quarterly meetings.
  • Submit monthly reports to SNP in a timely basis.
  • Perform duties as outlined in SNP Pharmacist Consultant Standards
QUALIFICATIONS:

  • Knowledge and expertise in drug therapy management of the geriatric and other long-term care patient/residents.
  • Knowledge of pertinent state and federal laws, regulations, and survey guidance pertaining to medications and pharmacy services related to long-term care facilities and patients/residents.
  • Must have and maintain current knowledge of drugs, drug information, drug interactions, plus Board of Pharmacy and DEA rules and regulations.
  • Maintain current knowledge in the field of pharmacy by attending seminars and workshops.
  • Possess a reliable means of transportation to satisfy travel requirements.
  • Ability to work independently to plan, develop and coordinate his/her consultant activities.
EDUCATION/EXPERIENCE:

1. Minimum requirements include a B.S. in pharmacy, PharmD. preferred.

2. Must be a licensed pharmacist in California.

3. Prefer a minimum of 3-4 years experience as a consulting pharmacist in a long-term care setting.

4. Must maintain a valid drivers license and be able to move freely among facilities.

5. Knowledge of, use, and practice of excellent customer services skills, including telephone skills.

6. Must be friendly, cooperative and a team player.

7. Must have a professional appearance.

8. Basic computer skills, proficient in required computer software.
