Our Community Health Nurses deliver care to patients in the community, providing a wide range of skilled nursing care while working to the top of their licensure. We are a comprehensive team comprised of RNs, Physical Therapists, LPNs, LNAs, SLPs, Social Workers and administrative support, all collaborating together to support patients to remain living in the setting of their choice, their home.

Requirements
- Greater than two years of home or public health nursing experience preferred, or a combination of education and experience from which comparable knowledge and skills are acquired.
- Previous home care desired but not required. We are willing to train and develop a candidate who demonstrates a willingness and desire to learn home health nursing.
- Graduate nurses are considered for our Community Health Nurse I level.
On Call: Not Required
Incentives: $20k Sign On Bonus, up to $20k in student loan forgiveness, and $2k/mo for 3mos housing stipend for external applicants, prorated for part time roles! Mileage reimbursed at Federal Gas Rates!
