Job Title: Social Worker – Reablement Team Location: Brent Rate: £32 per hour An exciting job role has recently become available at the Brent London Borough Council. They are looking for a new member for their Reablement Team. Responsibilities In this role, you will conduct comprehensive assessments of individuals’ needs, strengths, and goals. You will implement interventions aimed at promoting independence and reducing the need for long-term care. Additionally, you will work closely with healthcare providers, therapists, and community services to ensure a holistic approach to reablement.
