Performs intermediate professional work assessing difficult social service needs, counseling, and assisting clients, determining available programs and services, providing services to clients, and related work as apparent or assigned. Work is performed under the limited supervision of the assigned supervisor. Responsibilities Interviews clients and assesses needs; determines what programs or resources client is eligible to receive; completes applications for programs. Evaluates client's financial situation through review of income and expenses; provides alternatives to budget/planning to improve well-being. Refer and/or connect clients with additional resources (in-house or outside organizations or agencies) as appropriate. Submits vouchers for payment; makes payment to providers. Schedules appointments for home-bound/disabled clients. Develops a goal plan, develops strategies or a plan of action for clients, for crisis/problem resolution. Provides support, education, and counseling to families; provides budgeting education and offers suggestions for self-sustainability. Acts as a client advocate; provides transportation and/or accompaniment for appointments, services and/or activities identified in the client's goal plan. Monitors, reviews, and maintains direct client contact in both Intake and Ongoing Case Management. Performs other assigned duties. Qualifications Thorough knowledge of social, economic and health problems. Thorough knowledge of social institutions and the methods of the helping process. Thorough knowledge of individual and groups behavior. Ability to identify social problems and needs to assess the ability of individuals and families to utilize services in problem solving. Ability to plan and organize work and to understand and interpret laws, policies and regulations. Ability to solve problems within scope of responsibility. Ability to prepare reports and maintain records. Ability to establish and maintain effective working relationships with clients, associates, court staff, law enforcement staff, mental and medical health providers, social agencies, and the public. Education & Requirements Bachelor’s degree with coursework in social work, human services, or related field.
