Requirements: - Active CMT, CNA/M, or LPN/LVN certification/license.
- Experience in Long-Term Care or Skilled Nursing (preferred)
- Strong attention to detail and medication safety practices.
- Ability to work independently and as part of a team.
- High level of integrity, dependability, and compassion.
- Passion for caring for long-term care residents and skilled rehabilitation patients.
Responsibilities: - Administer medications via approved routes under RN supervision.
- Maintain medication carts and secure, organized storage areas.
- Obtain approval before administering PRN medications.
- Perform non-narcotic medication counts at shift change.
- Monitor medication supplies and coordinate reorders.
- Accurately document medications on the MAR.
- Obtain and record vital signs, including apical pulse.
- Observe and report changes in resident condition.
- Assist with resident care, including CNA duties.
- Communicate effectively with nursing staff, providers, and care teams.
- Maintain professionalism with residents and families.
- Work efficiently in a fast-paced environment with frequent interruptions.
